Brands and Affiliates Disclosure

This Brands and Affiliates Disclosure identifies the companies, subsidiaries, affiliated businesses, and consumer-facing marketing brands that operate within the Motiv Brands network.

Motiv Brands, Inc. is a Wyoming corporation and the parent company of the Motiv network. Motiv Brands, Inc. owns controlling interests in certain affiliated businesses, including Leads Motiv, LLC, and operates alongside its subsidiaries and controlled affiliates.

Many of the consumer-facing brands identified below are marketing names or DBAs used by Leads Motiv, LLC and other Motiv-affiliated companies to provide lead-generation and related consumer services.

Use of a particular Motiv brand does not necessarily mean that the brand itself is a separate legal entity.

Corporate Entities

The Motiv network currently includes the following corporate entities and subsidiaries:

Form Motiv

Credit Motiv

Trip Motiv

Additional subsidiaries or affiliated entities may be added to the Motiv network from time to time.

Consumer-Facing Brands and DBAs

The Motiv network currently uses the following consumer-facing marketing brands and DBAs:

Home Motiv — Home services

Insure Motiv — Insurance-related services, including home and life insurance

Disability Motiv — Social Security Disability-related lead generation and consumer matching

Windows Motiv — Window replacement services

Bath Motiv — Bath remodeling and shower installation services

Additional consumer-facing brands may be launched in the future.

This page will be updated as new brands are added, renamed, retired, or transferred within the Motiv network.

Relationship Between the Brands

Consumer-facing Motiv brands are generally used for marketing, lead generation, consumer education, service matching, and related commercial purposes.

Unless otherwise expressly stated:

a consumer-facing brand is not a separate legal entity;

the applicable website or service may be operated by Leads Motiv, LLC, Motiv Brands, Inc., or another controlled Motiv affiliate;

information collected through a Motiv brand may be processed by affiliated Motiv entities as reasonably necessary to provide the requested service, operate the platform, maintain compliance, perform analytics, or administer consumer rights;

the specific service provider ultimately selected to receive a consumer lead may be an independent third party and not part of the Motiv corporate family; and

participation in one Motiv brand does not automatically enroll a consumer in another Motiv service.

Lead-Generation Activities

Leads Motiv, LLC is the primary Motiv-affiliated entity that uses consumer-facing DBAs for lead-generation activities.

When a consumer submits a request through a Motiv brand, information may be used to:

evaluate the consumer's request;

determine potential eligibility;

identify an appropriate provider;

route the request through matching or ping/post systems;

connect the consumer with a selected service provider; and

facilitate communications relating to the service requested.

The completed lead is generally intended to be delivered exclusively to one selected service provider for the specific service requested, unless otherwise disclosed.

Cross-Brand Marketing

Subject to applicable law and the consumer's communications choices, Motiv may use ordinary contact and marketing information to inform consumers about other products or services available through the Motiv family of brands.

A selected third-party service provider is expected to use a Motiv lead for the specific service request for which it received the lead unless another use has been separately disclosed and authorized.

Motiv does not use specific Disability Motiv health-condition, treatment, medication, or disability questionnaire responses to determine unrelated cross-brand marketing offers.

Privacy and Consumer Rights

The collection, use, disclosure, and retention of personal information across the Motiv network are governed by our Privacy Policy and, where applicable, additional brand-specific privacy disclosures.

Certain Disability Motiv activities may also be subject to a separate Consumer Health Data Privacy Policy.

Updates to This Disclosure

Motiv may update this Brands and Affiliates Disclosure from time to time as:

new subsidiaries are formed or acquired;

brands are launched;

brands are renamed;

brands are retired;

ownership structures change; or

business operations change.
